Referral & Intake Process

Our intake process provides the first step toward effective, individualized services that will meet your specific needs and help you reach your wellness goals. To get you started, our Intake Team will gather your information, answer any questions you may have, and schedule you for an intake screening. Once your benefits are verified, you will receive confirmation of your appointment and an email to set up your client portal and complete necessary paperwork.

Intake Screening

• you will work with a clinician to determine what services, clinician and scheduling works best for you. which includes a 30-45 minute screening, which may be held in the office or via telehealth.

• we will also discuss your preferred appointment schedule, and your insurance coverage or payment options.
